The overall aim of this research project is to develop and evaluate a structured intervention in which physical activity is integrated into psychological treatment for individuals with stress-related exhaustion. The project includes studies examining physiological and psychological responses to physical activity, as well as individuals’ experiences of engaging in physical activity in everyday life. Findings from these studies have informed the development of the intervention and the subsequent feasibility study. The overarching goal is to generate knowledge on how physical activity can be used in a safe, sustainable, and effective way as part of rehabilitation.
The project includes three interrelated studies.
Study 1 examined physiological and psychological responses to single exercise sessions in individuals with stress-related exhaustion compared with healthy controls. The study was conducted in a laboratory setting, where participants completed two cycling sessions, one at low intensity and one at moderate intensity. Psychological and physiological responses were measured before, during, and after exercise.
Study 2 explored experiences of physical activity in everyday life through qualitative interviews with individuals with stress-related exhaustion. The focus was on factors that facilitate or hinder engagement in physical activity, as well as strategies participants use to manage activity during recovery.
Study 3 builds on findings from the first two studies and evaluates a treatment manual in a clinical feasibility study. Approximately 25 participants are recruited through healthcare providers such as occupational health services, primary care, and related settings where individuals with stress-related exhaustion receive treatment. Participants take part in an intervention consisting of approximately 5–6 sessions in which physical activity is integrated into psychological treatment delivered by clinicians with cognitive-behavioral competence within routine care. Outcomes related to fatigue, mental health, and physical activity are assessed before and after the intervention, and experiences of the intervention are collected from both participants and clinicians.
By developing and evaluating an intervention grounded in psychological and physiological knowledge and in the experiences of individuals with stress-related exhaustion, the project aims to inform more targeted rehabilitation strategies. Improved integration of physical activity into treatment may support recovery and functional capacity and thereby reduce the impact of stress-related exhaustion for both individuals and society. The study also provides a foundation for future effectiveness trials in which the intervention can be evaluated on a larger scale.
Detta forskningsprojekt har som mål att utveckla och utvärdera en strukturerad intervention där fysisk aktivitet integreras i psykologisk behandling för personer med stressrelaterad utmattning. Projektet omfattar studier av fysiologiska och psykologiska responser på fysisk aktivitet samt individers erfarenheter av fysisk aktivitet i vardagen, vilka har legat till grund för interventionens utformning och den efterföljande genomförbarhetsstudien. Den övergripande ambitionen är att bidra med kunskap om hur fysisk aktivitet kan användas på ett säkert, hållbart och effektivt sätt som en del av rehabilitering.
Projektet omfattar tre delstudier:
Delstudie 1. Den första studien undersökte hur personer med stressrelaterad utmattning reagerar på fysisk aktivitet jämfört med friska personer. Studien genomfördes i laboratoriemiljö och deltagarna genomförde två cykelpass, ett på låg och ett på måttlig intensitet. Psykologiska och fysiologiska reaktioner mättes före, under och efter träningspassen.
Delstudie 2. Den andra studien har genom kvalitativa intervjuer undersökt hur personer med stressrelaterad utmattning upplever fysisk aktivitet i vardagen. Fokus låg på vilka faktorer som underlättar eller försvårar att vara fysiskt aktiv, samt vilka strategier deltagarna använder för att hantera aktivitet under sin återhämtning.
Delstudie 3. Resultaten från de två första delstudierna ligger till grund för utvecklingen av en behandlingsmanual, som utvärderas i den tredje delstudien i form av en genomförbarhetsstudie i klinisk verksamhet. Cirka 25 deltagare rekryteras via vårdgivare såsom företagshälsovård, primärvård och närliggande verksamheter där personer med stressrelaterad utmattning får behandling, och deltar i en intervention bestående av cirka 5–6 sessioner där fysisk aktivitet integreras i psykologisk behandling. Interventionen genomförs av behandlare med KBT-kompetens inom ramen för ordinarie vård. Utfall relaterade till trötthet, psykisk hälsa och fysisk aktivitet mäts före och efter insatsen, och erfarenheter av interventionen samlas in från både deltagare och behandlare.
Genom att utveckla och utvärdera en intervention som bygger på både psykologisk och fysiologisk kunskap och erfarenheter från personer med stressrelaterad utmattning kan projektet bidra till mer träffsäkra rehabiliteringsinsatser. En bättre integrering av fysisk aktivitet i behandling kan stödja återhämtning och funktionsförmåga, och därav minska konsekvenserna av stressrelaterad utmattning för både individ och samhälle. Studien lägger även en grund för framtida effektstudier där interventionen kan prövas i större skala.

The overall aim of this study is to study the physiological and psychological effects of physical activity on patients with fatigue syndrome. The project consists of two sub-studies.
In study one, the effects of different intensity levels of physical activity on physiological and psychological mechanisms in patients with fatigue syndrome are studied in comparison with healthy individuals. The data collection started in spring 2023.
In study two, the researchers will study the effect of physical activity as an adjunct to standard CBT treatment for patients with exhaustion in a randomized controlled trial. This sub-study is planned to start in 2024.
Three main research questions are in focus:
- What are the immediate physiological and psychological effects of different doses of physical activity in patients with fatigue syndrome?
- What are the rehabilitation effects of individualized physical activity in addition to standard CBT in the treatment of fatigue syndrome?
- How is physical activity experienced in patients with fatigue syndrome?
