This research project aims to strengthen collaboration between central actors in the rehabilitation process, with a particular focus on the role of the employer. By developing and testing a digital employer support system, we want to facilitate dialogue, planning and work adaptation for people who are on sick leave due to stress-related mental illness. The goal is to create better conditions for collaboration and return to work.
We want to investigate how support for strengthened collaboration between employer and patient can be used in practice. Which parts are perceived as most useful? Is the dialogue with the employee strengthened? Are roles and responsibilities clearer? And can the support help employers identify and manage factors in the work environment that affect the return to work, such as workload, support and a collaborative climate?
The study is being conducted as a so-called feasibility study where we test the digital support on a small scale for six months. We intend to recruit 20–30 people on sick leave with stress-related mental health problems and their immediate managers. Participants are recruited via primary care and the Swedish Social Insurance Agency. Both employees and managers fill out digital surveys on three occasions (start, 3 months and 6 months), and a selection is interviewed to provide an in-depth understanding of experiences, obstacles and suggestions for improvement.
The digital support is easy to use, does not require logging in and contains practically applicable material such as dialogue support, templates and guidance. The content is designed to support the entire rehabilitation process – from the first contact with the person on sick leave to collaboration meetings with the Swedish Social Insurance Agency and other stakeholders. Managers are also offered limited guidance by the research team if necessary.
By developing support that strengthens the role of the employer and improves collaboration between actors, we hope to contribute to a more cohesive and effective rehabilitation process. The goal is for more people to be able to return to work in a sustainable way, which benefits both the individual's health and society's resources. The study also lays the foundation for a future impact evaluation where the digital support can be tested on a broader scale.

Stressrelaterad psykisk ohälsa är idag en av de vanligaste orsakerna till sjukskrivning i Sverige. Trots kunskap om sambandet mellan arbetsmiljö och psykisk hälsa är tillgången till effektiva rehabiliteringsinsatser fortfarande otillräcklig. En särskild utmaning är att den rehabilitering som erbjuds ofta kräver att flera olika aktörer – såsom arbetsgivare, Försäkringskassan och vården – samverkar. Men samverkan i praktiken präglas ofta av oklara roller, bristande kommunikation och otillräckligt stöd till arbetsgivare, vilket kan försena återgång i arbete.
Projektets syfte
Det här forskningsprojektet syftar till att stärka samverkan mellan centrala aktörer i rehabiliteringsprocessen, med särskilt fokus på arbetsgivarens roll. Genom att utveckla och testa ett digitalt arbetsgivarstöd vill vi underlätta dialog, planering och arbetsanpassning för personer som är sjukskrivna på grund av stressrelaterad psykisk ohälsa. Målet är att skapa bättre förutsättningar för samverkan och återgång i arbete.
Vad ska vi undersöka?
Vi vill undersöka hur stöd för stärkt samverkan mellan arbetsgivare och patient kan användas i praktiken. Vilka delar upplevs som mest användbara? Stärks dialogen med medarbetaren? Blir roller och ansvar tydligare? Och kan stödet hjälpa arbetsgivare att identifiera och hantera faktorer i arbetsmiljön som påverkar återgången i arbete, såsom arbetsbelastning, stöd och samarbetsklimat?
Hur går studien till?
Studien genomförs som en så kallad genomförbarhetsstudie (feasibility study) där vi i liten skala testar det digitala stödet under sex månader. Vi ämnar rekrytera 20–30 sjukskrivna personer med stressrelaterad psykisk ohälsa samt deras närmaste chefer. Deltagarna rekryteras via primärvården och Försäkringskassan. Både medarbetare och chefer fyller i digitala enkäter vid tre tillfällen (start, 3 månader och 6 månader), och ett urval intervjuas för att ge fördjupad förståelse av upplevelser, hinder och förbättringsförslag.
Det digitala stödet är enkelt att använda, kräver ingen inloggning och innehåller praktiskt tillämpbart material såsom dialogstöd, mallar och vägledning. Innehållet är utformat för att stödja hela rehabiliteringsprocessen – från första kontakten med den sjukskrivna till samverkansmöten med Försäkringskassan och andra aktörer. Chefer erbjuds även begränsad vägledning av forskargruppen vid behov.
Samhällsnytta
Genom att utveckla stöd som stärker arbetsgivarens roll och förbättrar samverkan mellan aktörer hoppas vi bidra till en mer sammanhållen och effektiv rehabiliteringsprocess. Målet är att fler ska kunna återgå i arbete på ett hållbart sätt, vilket gynnar både individens hälsa och samhällets resurser. Studien lägger också grunden för en framtida effektutvärdering där det digitala stödet kan testas i bredare skala.
Detta projekt ingår i forskningsmiljön E-PABS – ett excellenscenter för fysisk aktivitet, hjärnhälsa och hållbarhet.
About the project
Stress-related disorders are today one of the most common causes of sick leave in Sweden. This research project investigates how digital employer support can strengthen the dialogue and collaboration between employers, employees on sick leave and the Swedish Social Insurance Agency – to facilitate a sustainable return to work.
The study is carried out by researchers at the Swedish School of Sport and Health Sciences, GIH, and is funded by FORTE and the Swedish Social Insurance Agency (Försäkringskassan).
Research approach
The project is carried out in two sub-studies that together contribute to the development and evaluation of digital employer support.
Studie 1 – Qualitative studie (completed)
In the first sub-study, approximately 20 representatives of employers, healthcare professionals and the Swedish Social Insurance Agency (Försäkringskassan) were interviewed. The aim was to identify obstacles and opportunities in the current collaborative process in the rehabilitation of stress-related disorders. The results showed the need for clearer roles, better communication and more structured support for employers.
Studie 2 – Feasibility study (ongoing)
In the second sub-study, 40–60 participants – people on sick leave with stress-related health issues and their managers – participate in testing the digital employer support over a six-month period.
The data collection consists of surveys and interviews and aims to investigate:
- how useful, relevant and helpful the support is perceived to be,
- how it affects dialogue, planning and collaboration between the actors, and
- which factors facilitate or hinder its use in practice.
The experiences from this feasibility study will form the basis for a future larger efficacy study (RCT).
Background of the project
An increasing number of people are affected by stress-related disorders, which leads to extensive human and socio-economic consequences. Rehabilitation for these conditions often requires collaboration between several actors – employers, health and medical care and the Swedish Social Insurance Agency – but research shows that coordination is often lacking.
Previous studies have shown that the role of the employer is crucial for the success of the return to work. At the same time, research indicates that many managers experience uncertainty about how to act.
There is therefore a great need for concrete and easily accessible support that strengthens the employer's participation in the rehabilitation.
Purpose
The purpose of the project is to develop and test a digital employer support that will:
- facilitate collaboration between employers, employees and the Swedish Social Insurance Agency,
- increase the employer's security and competence in rehabilitation work,
- contribute to a more structured and sustainable return to work for stress-related disorders.
Societal relevance and benefits
The project contributes with new knowledge about digital collaboration, workplace-oriented rehabilitation and the employer's role in stress-related disorders. The results are expected to provide a basis for future randomized controlled trials (RCTs) and the development of scalable digital support within the welfare system.
